Advanced cancer genomics for Southwest Virginia and beyond.

The Molecular Diagnostics Laboratory at the Fralin Biomedical Research Institute is a CLIA-certified, CAP-accredited academic laboratory focused on the molecular characterization of cancer. Clinical testing, assay validation, and translational research operate within a single institutional structure and in coordination with regional healthcare partners.

 

A regional academic laboratory for cancer molecular diagnostics 

The laboratory integrates advanced genomic technologies, analytical rigor, and collaborative expertise to expand access to molecular characterization of cancer and support precision oncology research. Clinical molecular diagnostics, assay development and validation, and translational research are pursued within a single institutional structure.

Genomic Capabilities

The laboratory addresses multiple classes of genomic alteration in cancer through targeted next-generation sequencing, digital PCR, and, in a research-use context, long-read sequencing.

Alteration classes

  • Single-nucleotide variants (SNVs)
  • Small insertions and deletions (indels)
  • Copy-number alterations (CNAs)
  • Gene fusions
  • Structural variants
  • Microsatellite instability (MSI)*
  • Tumor mutational burden (TMB)*

Clinically reportable   The Oncomine Myeloid Assay is analytically validated and clinically reportable. Results carry clinical significance in myeloid malignancies as interpreted by the treating clinician.
Under analytical validation   The Oncomine Comprehensive Assay Plus is under analytical validation and is not currently clinically reportable.
Research-use platforms   Long-read sequencing on the Oxford Nanopore GridION and biomolecular interaction analysis on the Biacore 3000 are used in a research-use context and do not generate clinical reports.

* MSI and TMB are calculable from broad-panel sequencing. Their inclusion in the clinical menu is contingent on completion of validation of the corresponding assay.

Quality and accreditation

CLIA certification. The laboratory operates under CLIA certification #49D0016288 for high-complexity clinical testing.

CAP accreditation. The laboratory holds accreditation under the CAP Laboratory Accreditation Program.

Accreditation applies to the laboratory as a testing entity and does not, in itself, endorse any specific assay. Individual clinical assays are made available only after in-house analytical validation.

A regional resource

The cancer landscape in Southwest Virginia reflects both national trends and regional factors. Cancer incidence and mortality rates are above state and national averages, particularly for lung, colorectal, breast, and endometrial cancers, shaped by regional epidemiological and socioeconomic conditions.

An academic molecular diagnostics laboratory located within the region shortens the geographic distance between specimen and analysis and situates molecular characterization within the same clinical and research community it serves.

Research-use characterization of tumor genomes, including long-read sequencing of structural variants and biomolecular interaction studies.

Implementation science
Studies of how molecular diagnostic testing is incorporated into clinical workflows in regional healthcare.

Research-use activities are governed separately from clinical testing operations and do not enter clinical care.

LEADERSHIP

Dr. Carla Finkielstein is Professor of Cell and Molecular Biology in the Department of Biological Sciences at Virginia Tech and the Fralin Biomedical Research Institute and is the Scientific Director of the Molecular Diagnostics Laboratory, where she oversees the development, validation, and clinical performance of molecular tests supporting precision medicine across diverse specimen types. She also directs the Integrated Cellular Responses Laboratory, leading a multidisciplinary team of biochemists, clinicians, mathematicians, and engineers to dissect how circadian disruption shapes cancer initiation and progression.

With nearly three decades of experience as a scholar and researcher, Dr. Finkielstein has authored more than 90 peer-reviewed publications and book chapters, filed and commercialized multiple patents, and received numerous awards and recognitions throughout her career. She holds affiliated appointments at the Virginia Tech–Carilion School of Medicine and the Department of Surgery, and serves as Associate Director of Research for the Academy of Integrated Sciences. Dr. Finkielstein is a member of several professional associations, a reviewer for scientific journals within her field of specialization, and a panelist for U.S. federal and international funding agencies. She earned her Ph.D. from the University of Buenos Aires and completed postdoctoral training at the University of Colorado Health Sciences Center.

Douglas J. Grider, M.D., is a Diplomate of the American Board of Pathology in Dermatopathology, Anatomic Pathology and Clinical Pathology.

Dr. Grider completed medical school at the Edward F. Hebert School of Medicine, Uniformed Services University, Bethesda, Maryland, followed by a pathology residency at Wilford Hall USAF Medical Center, San Antonio, Texas. He completed an anatomic pathology fellowship during his last year of residency, and he later went on to complete both a gastrointestinal pathology fellowship and a dermatopathology fellowship at the Armed Forces Institute of Pathology in Washington D.C.  After a 24-year Air Force career, Dr. Grider joined DPA in 2010 and currently is the managing partner.

Dr. Grider is an associate professor at the Virginia Tech Carilion School of Medicine (VTCSOM). He advises VTCSOM medical students about the discipline of pathology and pathologist careers. He is the founding and current dermatopathology director for the VTCSOM dermatology residency, and pioneered the integration of pathology into the VTCSOM Gastroenterology and Hepatology Fellowship.
