Parkinson’s disease (PD) affects 1% of the population over age 60, making it the second most common neurodegenerative disorder and disease prevalence is only expected to rise. The insidious nature of PD implies the disease progresses through phases. In fact, by the time the disease is diagnosed, moderate loss of dopaminergic neurons within the substantia nigra has already occurred. Given that targeted therapies have failed to modify the course of the disease, a paradigm shift has occurred recently to apply treatments at an earlier stage. This demands the accurate identification of those at greatest risk, or in a prodromal phase of the disease. In this seminar, we will define that prodromal period, summarize novel risk factors for the development of PD through leveraging national Veterans Affairs health data, and describe imaging characteristics that may predict progression to manifest disease. All of this will set the stage for upcoming neuroprotective trials.
